Scheduling Your Appointment

Getting started is simple. Click “Contact Us” to send a message about what you’re looking for, or to request a complimentary 15-minute introductory phone call to discuss your needs and see if Restore Mental Health is a good fit. We strive to respond within 2–3 business days.

Once we’ve connected and you’re ready to schedule, you’ll receive instructions for accessing your secure client portal.

Complete Your Intake Forms

Before your first appointment, please log in to the client portal to complete your intake forms, health questionnaires, and practice documents. Completing these before your visit allows us to spend more of our time together focused on understanding your concerns and developing a personalized treatment plan.

The client portal is also where you can securely manage appointments, complete forms, and communicate with the practice.

Preparing for Your First Visit

To help us make the most of your appointment, please have the following available:

  • A list of current and past medications and dosages
  • A list of current and past interventions such as supplements, psychotherapy, TMS, etc.
  • Relevant medical records, laboratory results, or prior psychiatric records (if available)
  • A list of questions or goals you’d like to discuss

For patients under 18 years of age, a parent or legal guardian must participate in the initial evaluation.

What You Can Expect

  • Comprehensive psychiatric evaluations are scheduled for 60 minutes.
  • Appointments are available in person and via secure telehealth throughout Hawaii.
  • Treatment recommendations are developed collaboratively and tailored to your individual needs and goals.
  • Follow-up appointments are scheduled based on your treatment plan and adjusted over time as your needs change.
